Claiming Rules and Procedures

1. Claiming will be done on February 6, 2024 – Tuesday – from 10am to 12 noon. At the SPMAFI office in SPUM.
2. Winners who can not personally claim their prize should send a representative and present the following:
   1. Valid ID of Winner
   2. Letter of Authorization (authorizing representative to claim prize)
   3. Half of the Stub.
3. Prizes not claimed 30 days after February 6, 2024, will be forfeited in the name of SPMAFI.
4. Once claimed, prizes are transferable.
5. Prizes are not convertible to cash.
